  10. Date of each lesson.
  11. Signature of the person undergoing training.

The example above shows how a safety training log is generally completed. If the entry for one of the columns does not fit on one line, the next one is used. If the column remains empty, put a dash.

Important! At the end of the briefing, an oral check is carried out, the results of which are reflected in the journal.

The beginning of the next year in the table is separated by the insert “Year XXXX”; dashes are placed on both sides of this mark. Then, recording continues from the first number.

Responsibilities of workers and employers regarding safety

Who is responsible for maintaining the journal?

Keeping a journal is necessary for several reasons. This allows you to track the list of employees who have been familiarized with safety rules, and therefore may be allowed to perform work associated with risk. Also, thanks to the reflection in the log, you can analyze the types and procedures for conducting instructions on how to use dangerous equipment.

The execution of the document is entrusted to the employee responsible for labor protection. Each enterprise appoints such an employee. This is done by issuing an internal order from the manager. All employees involved in this area are notified of the decision.

The journal is formatted as follows:

  1. Registered on the case list form.
  2. They are stitching.
  3. Number the pages.
  4. They put the seal of the person (or department) responsible for maintaining the document.

Entries are entered into the journal without interruption. When the new year begins, their serial numbers are reset to zero, and the marks begin again with the number “1”.

How is filling controlled?

Within a small enterprise, this responsibility rests with management or a designated responsible employee. OHS logs are used to systematize the necessary information, exercise operational control in the field of OHS at the enterprise and record information on compliance with deadlines for mandatory personnel briefings.

The list of occupational safety magazines that an enterprise must have depends on the specifics of its business activities, as well as which specialists are involved in the production process. However, there are also such registers, the presence of which is mandatory in any organization.

  1. Accounting:
    • Work according to orders and permits when working with electrical installations. It is used to display information on work carried out by specialists who have the appropriate rank and have undergone special instructions and knowledge testing.     • Medical examinations of drivers before flights. The register records information based on the results of medical examinations of drivers, based on the form presented in clause 5 of Appendix No. 9 to the order of the USSR Ministry of Health “On improving the medical examination system...” dated September 29, 1989 No. 555.
    • Issuance and maintenance of PPE (personal protective equipment). The recommended form of the journal is presented in Appendix No. 1 to the instructions, approved. by order of the Ministry of Energy dated June 30, 2003 No. 261.
    • Maintenance and repair work on real estate.
  2. Registrations:
    • permits to perform high-risk work;

Rules for filling out the OT magazine, how to lace the magazine

Until 03/01/2017, maintaining and, accordingly, filling out occupational safety journals at enterprises was carried out according to the GOST 12.0.004-90 standard. Now the procedure for maintaining occupational safety logs is used, adjusted to meet the requirements of GOST 12.0.004-2015 (Order of Rosstandart “On putting into effect...” dated 06/09/2016 No. 600-st).

All pages in the OT magazine must be numbered, and the numbering is used consecutively, starting from the title page. The title page, as a rule, indicates the name of the enterprise, structural unit (service, section), start and end dates of journal keeping.

The remaining thread after lacing all the pages of the magazine must be brought to the last page, fixed and sealed on top with a sheet of paper, on which it is necessary to indicate how many pages are numbered and laced. The inscription made on a strip of paper sealing the ends of the thread must be certified by the signature of the head of the enterprise and a seal (if any). When certifying with a seal, it should be taken into account that its imprint should be imprinted both on the pasted strip and on the last page of the laced journal.

How to keep a log of OT orders, how to flash it correctly

A journal of occupational safety orders is required at enterprises with operating production workshops (sites) to record all orders issued by management related to:

  • to employee training;
  • permission to perform work independently;
  • transfer of powers to carry out orders to responsible persons;
  • payment of incentives, announcement disciplinary sanctions etc.

The log must necessarily contain records of the familiarization of these orders by the employees mentioned in them. After the end of the calendar year, a thick line is drawn under the entries made for the past year and marked next year. The numbering begins again.

Like all registers established at enterprises, the journal of occupational safety orders must be numbered (starting from the title page) and laced before use. The thread with which the magazine is laced must be placed on its last page and covered with a strip of paper with an inscription indicating the number of pages laced. The inscription is certified by the signature of the person responsible for maintaining the register, the head of the enterprise, as well as a seal - in this case, one part of the imprint should be located on a glued piece of paper, and the other on the last page of the journal (GOST R 7.0.8-2013).

How to properly prepare a journal for issuing occupational safety certificates

The form of such a journal is not regulated by law, so it can be issued in the form of an ordinary A4 book printed in a printing house (taking into account Article 10 of the Law “On Accounting” dated December 6, 2011 No. 402-FZ, GOST R 7.0.8‑2013).

All columns in the journal must be filled in; missing lines and blots are unacceptable. Corrections can be made in the usual way in office work - by crossing out and writing the correct inscription next to it, certified by the signature of the employee responsible for maintaining the journal.

Before you start filling out, all pages of the magazine must be numbered, starting with the title, and stitched. The title page indicates the name of the enterprise, the start and end dates of its operation.

Inside the magazine is laid out in such a way that it is possible to indicate:

  • date of issue of the certificate;
  • number;
  • Full name of the employee to whom the certificate is issued;
  • job title;
  • place of work ( structural subdivision, Department);
  • notes.

According to the provisions of section 7.3 of the list, approved. By order of the Ministry of Culture dated August 25, 2010 No. 558, occupational safety magazines have different shelf life, depending on the information entered in them. Thus, accident logs must be kept permanently (clause 630 of the List). Safety briefing logs are kept for 10 years (clause 626). Logs of registration of administrative penalties for violation of sanitary standards, as well as records of decisions on fines - 3 years (paragraphs 642 and 643).

The enterprise, in accordance with GOST 12.0.004-90, conducts the following safety training:

Re-briefing should be carried out with all employees of the enterprise at least once every six months. It is carried out by a safety engineer or the head of a structural unit. Its main purpose is to test the existing knowledge of employees and, if necessary, fill gaps.

Unscheduled briefing must be carried out with all employees of the company when new labor protection standards come into force, new equipment is introduced or changes production process, identification of facts of safety violations, in case of accidents and emergencies, at the request of inspection bodies, etc. It is conducted by a safety engineer or the head of a structural unit in the scope and subject matter, depending on the reason for this training.

Targeted training is the acquisition of knowledge on a specific one-time function (for example, emergency response, etc.)

Who is assigned responsibility and who keeps the log

In accordance with the law, the safety manager and engineer must have valid certificates that they have been trained in specialized institutions. At the same time, it is necessary to remember that the manager must be trained once every 5 years, because only he is responsible for organizing safety and health at the enterprise.

Procedure for keeping a TB journal and sample filling

The safety briefing log must be kept continuously throughout the enterprise. standard form in compliance with the mandatory details provided for by GOST 12.0.004-90.   • Notes in the briefing log must necessarily be preceded by appropriate measures, namely, the holding of the briefing itself by the responsible person. Only after familiarization with all the rules and safety standards that are established at the enterprise, the appropriate notes are entered into the log.
  • For more convenient management, experts recommend keeping a separate journal for each type of instruction. Thus, information will be stored in a more accessible and understandable form.
  • All items that are not filled out in the journal because they are unnecessary must be filled in with dashes. You can't leave them empty. Empty columns in a document may arouse suspicion among regulatory authorities.
  • If the journal is kept for more than one year, then each of them must be separated from the other. For this purpose, a special designation is used - the entire column is indicated this year, and the remaining voids are filled with a horizontal line: ———— 2017————
  • In each new year, the numbering of the serial numbers of those instructed should begin with 1.

As a rule, the working page of the magazine consists of 12 items placed in one row on the spread. They are filled in in the following order:

How often should I fill it out?

It is quite difficult to determine any specific frequency of filling out the safety log. This is due to the fact that each enterprise has its own events with a certain frequency. In addition, there are different kinds briefings that can be carried out outside any regime.

  • two dates that correspond to the beginning of filling out the journal and the end of all entries in it.

This log should display the following information:

  • date of completion, indicated in the format dd,mm.yy;
  • year of birth of the instructed employee;
  • his full name;
  • position held in the organization;
  • the profession of an employee of the organization, and in the case of a posted employee, his main place of work and identification details are indicated;
  • name and number of the instructions that are used in this case;
  • FULL NAME. the employee conducting the training;
  • personal signatures of the person being instructed and the person instructing;
  • work experience in a new place;
  • in case of an extraordinary briefing, the reason for it is indicated;
  • number of shifts;
  • FULL NAME. the person who issued permission to perform certain work at this enterprise;
  • the position held by the employee eligible for introduction;
  • date of issue of the permit;
  • personal signatures of the person who received the permit and the person who issued it.

The journal is kept on A4 sheets, where the data to be filled is located in a line. If necessary, one entry can occupy several lines. In the empty spaces left after filling, it is necessary to make dashes. Any commission checking the journal maintenance may make a remark in their absence.

Responsibilities of employee and employer

Both the head of the organization and its employees have some job responsibilities concerning the safe conduct of work, including instructions on them. The employee is obliged:

  • comply with all requirements when conducting hazardous work established by a number of regulations and other documents;
  • undergo timely training on the safe performance of the required types of work, which must be reflected in the necessary journal entry;

  • carry out all necessary activities for conducting safety training;
  • take all measures to complete the internship at the workplace;

  • do not allow dangerous types of work to be performed by people who have not been familiarized with and have not signed the safety log;
  • promptly check your employees for knowledge of the rules and regulations when performing hazardous types of work;
  • inform your employees in advance about possible risk for their health when performing certain work, as well as to provide them with all necessary means protection.
